The Lower Columbia Community Health Centre grew out of a community-led response to health care challenges in the Lower Columbia region. Over the past four years, dedicated volunteers from Rossland, Warfield, Trail, Montrose, Fruitvale, and Areas A and B of the Kootenay Boundary Regional District came together to advocate for a new, community-based model of
care.

Through regional visioning sessions, planning, and the formation of a not-for-profit society, this work led to Ministry of Health funding approval in September 2024. With extensive volunteer effort, the Health Centre began operations in July 2025, held a soft opening in August, and officially opened its doors in September 2025. We are proud to now be serving our community.

These questions help explain who we are and what to expect from the Lower Columbia Community Health Centre.

A Community Health Centre (CHC) is a not-for profit, community governed health care organization focused on team-based primary care, health promotion, and addressing factors that affect well being. It is guided by the needs of the community it serves.

The centre was created because existing care options were not meeting local needs. The CHC model provides a sustainable way to bring team-based care to the Lower Columbia region.

Yes. Once you are a registered patient, you will continue to see the same doctor or nurse practitioner. You also have access to other team members based on your needs.

No. We are not a walk-in clinic. Care is provided by scheduled appointment with registered patients.

As we grow, we will accept new patients from Kootenay Boundary’s Health Connect Registry (KBHCR.ca). If you currently have no provider, please register with the HCR by phoning 811.
